If you have not paid your renewal fees in time your name stands removed from the register under section 33. Upon payment of the renewal fees along with the defaulter charges and submitting required documents your name can be re-entered in the register of pharmacists. You can download form 13 for re-entry along with identity slip and submit same to the office (in person) with following documents.

1. Original Registration Certificate
2. Identity slip duly attested by a gazetted officer
3. 3.5 x 4.5 cm size six front posed recent identical photographs
4. Proof of Date of Birth
5. Proof of Residence

If you are a defaulter for more than last 10 years you are required to remain present before the executive committee
