How to obtain Pharmacist's Professional Profile Card?

To obtain a Pharmacist's Professional Profile Card, download the following application form and submit the application form in person at MSPC office along with the following documents.

1. Cash /D.D. of RS.200/-towards PPP charges
2. Xerox copy of Registration Certificate (front back). (Please bring your original Registration Certificate for verification)
3. Two front pose recent photographs size 4x6cms.
4. Proof of date of birth (if not recorded In Registration Certificate.)
5. Xerox copy of recent renewal receipt.
