How to renew Pharmacist's Professional Profile Card?

To renew a Pharmacist's Professional Profile Card, download the following application form and submit the application form in person at MSPC office along with the following documents.

Cash /D.D. of RS.100/-towards PPP charges
Xerox copy of Registration Certificate (front back). (Please bring your original Registration Certificate for verification)
Two front pose recent photographs size 4x6cms.
