How to notify change of address?

Write a letter addressed to the Registrar quoting your registration number and complete postal address along with pincode signed by you. You will receive a letter confirming your changed address.

How to notify change of name?

For female registered pharmacists

1. Original Marriage Certificate & one xerox.
2. Change of name form duly filled in.(download the same below)
3. Hand written application addressed to the Registrar.
4. Four 4 X 6 cm size, identical, front pose photographs.
5. Identity slip duly attested by gazetted officer / Principal of school or college
6. Surrender original Registration Certificate.
7. Address slip duly filled in.
8. Remit Rs. 220/- (Provided registration is renewed up to date)
9. Proof of date of birth.

For Male registered pharmacists(submission in person is must)

1. Affidavit on Rs. 100 stamp paper duly notarised for changed name
2. Gazette notification
3. Document showing changed name (ration card, driving license, passport, salary slip etc.)
4. Change of name form duly filled in.(download the same below)
5. Hand written application addressed to the Registrar.
6. Four 4 X 6 cm size, identical, front pose photographs.
7. Identity slip duly attested by gazetted officer / Principal of school or college
8. Surrender original Registration Certificate.
9. Address slip duly filled in.
10. Remit Rs. 220/- (Provided registration is renewed up to date)
11. Proof of date of birth.
