How to obtain a duplicate copy of registration certificate?

In case of loss of your registration certificate you can obtain a duplicate copy of registration certificate by submitting the following documents at MSPC office:

1. Copy of Police Complaint.
2. Affidavit from notary on Rs. 100/- Stamp Paper.
3. Hand written application addressed to the Registrar.
4. Four recent, identical, front pose photographs size 4 x 6 cms.
5. Identity slip duly attested by gazetted officer.
6. Address slip duly filled in.
7. Remit Rs. 200/- (Provided registration is renewed upto date)
8. Proof of date of birth.
9. Xerox copy of the Registration Certificate, if available.
