How to obtain a new copy of registration certificate?

In case your registration certificate is spoilt or mutilated you may obtain a new copy of your registration certificate by submitting the following documents at MSPC office:

1. Surrender Original Registration Certificate.(in whatever shape)
2. Hand written application addressed to the Registrar.
3. Four recent, identical, front pose photographs size 4 x 6 cms.
4. Identity slip duly attested by gazetted officer.
5. Remit Rs. 200/- (Provided registration is renewed upto date)
6. Proof of date of birth.
